Evidence
Start with what the source establishes. Preserve the distinction between a payer response, an account note and an interpretation. An action should be traceable to the information that supports it.
Action
State the next justified step and any information still needed. Describe the action precisely enough that another person could understand why it was chosen, without assuming every account with the same label needs the same response.
Ownership
Name the person responsible for continuing the work, the condition for completion and the next review. A handoff is incomplete when the receiver cannot tell what decision they are being asked to make.
